Title: LTSS Care Navigator

Location: Remote TX

Duration: 6 Months

Position Purpose:

Develops, assesses, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Develops or contributes to the development of a personalized care plan/service plan for members and educates members and their families/caregivers on services and benefit options available to improve health care access and receive appropriate high-quality care through advocacy and care coordination.

Candidate Requirements
Education/Certification	Required: High School/GED	Preferred: Bachelor's degree in Social Work or related field
Licensure	Required: LPN, LVN, or LCSW preferred. License is the most important.	Preferred:
Years of experience required Disqualifiers Best vs. average Performance indicators	Required Certifications: LPN, LVN or LCSW preferred. Bachelor's degree in Social Work, Health Services, Behavioral Science, or related field, or equivalent experience. 2+ years of experience in a managed care environment or working with people who have disabilities and/or with vulnerable populations who have chronic or complex conditions. Other state specific requirements may apply
