Job Details Patient Access and Collaborative Advanced Practice Clinician at Jefferson Family Medicine AssociatesDepartment of Family & Community Medicine, Sidney Kimmel Medical College at Thomas Jefferson University The Department of Family & Community Medicine at Thomas Jefferson University is seeking a Certified Registered Nurse Practitioner (CRNP), Physician Assistant-Certified (PA-C) or equivalent with primary care experience for a full-time, hybrid position that combines team-based care, in-basket medical decision-making and same-day clinical care delivery. This innovative role is central to Jefferson Family Medicine Associates' (JFMA) mission to improve access and advance high-quality, team-based care. About the Practice JFMA is the academic flagship site of Jefferson's nationally recognized Department of Family & Community Medicine, serving over 30,000 diverse patients. Our team is deeply committed to health equity, interprofessional collaboration, and continuous innovation in care delivery. About the Role The Hybrid Clinical and Inboxology APC will: *Provide EPIC in-basket support to JFMA clinicians based on a single care team, including collaborative medical decision-making tasks and resolving patient clinical concerns. *Deliver face-to-face primary care for scheduled follow-up, same-day sick/problem-focused visits, both in-person and via telehealth. *Collaborate closely with attending physicians, RNs, MAs, and schedulers to ensure high-quality and timely patient care. *Exercise autonomous clinical judgment with clear, thorough, and timely documentation. *Contribute to ongoing improvement of triage protocols and team-based workflows. *Participate in structured evaluation as part of a department-wide innovation pilot focused on workload optimization, access, and sustainability. *Have a minimum 2 years of primary care experience with strong clinical acumen in chronic disease management and common acute outpatient conditions. *Have some familiarity with Epic EMR; ability to manage time effectively across clinical and administrative responsibilities. Job Description Work Structure Full-time role (40 hours, with 50% of time devoted to face-to-face or telehealth care). Structured weekly schedule includes protected time for in-basket coverage, availability for urgent care visits, and administrative work. Participation in clinician/faculty development, team meetings, and pilot evaluation activities is expected. Why This Role?
