Community Program Liaison

Behavioral Health Services - CARS Intake Team

Hourly Pay Range: $26.61 - $39.91

Job Summary: The CARS Intake Team Community Program Liaison assesses client needs, determines service eligibility, and manages program access through intake and referrals. Duties include developing care plans, monitoring progress, assessing suicide risk (C-SSRS) and risk factors (SAFE-T), and creating crisis response plans. This position collaborates with referral sources, develops Conditions of Release plans, makes referrals, provides interim care coordination, and ensures Civil Commitment standards and follow-up.

Essential Responsibilities:



  • Determine client needs, eligibility, and medical necessity for services, including Case Management, Community Recovery Services, Outpatient, Day Treatment, Peer Support, and Recovery Support. Manage program access, intake, and referrals. Develop care plans, monitor client progress, and assess and develop crisis response plans.
  • Collaborate with referral sources and develop Response and Conditions of Release plans per state guidelines. Make referrals based on plans and comprehensive client needs. Provide interim care coordination as needed. Ensure Civil Commitment planning standards and follow-up.
  • Assess for client suicide risk through use of C-SSRS. Address client risk factors when clinically indicated with use of SAFE-T. Consult with supervisor, lead, or Intake Team when necessary.
  • Utilize effective oral and written communication skills and organizational skills. Demonstrate the ability to work effectively and harmoniously with staff, clients, families, community organizations, and the public.
  • Perform all other duties as assigned.



Minimum Requirements:



  • Current resident of Wisconsin.
  • Possession of a valid Wisconsin Driver's License.


  • Possession of a Bachelor's Degree or higher from an accredited college or university.
  • At least three (3) years of professional experience with mental health and/or substance use disorders.
  • Intermediate knowledge of all related computer and software applications, such as word processing and spreadsheets.
  • Knowledge of federal, state, and county regulations related to the provision of mental health services.
  • Willing and able to travel within the community up to 25% of the time in this role.
