Title: Resource Specialist

Duration: 03 Months (Contract to Hire )

Location: Region 6 - Tampa Bay Area (Hardy, Highlands, Hillsborough, Manatee, & Polk)


Individual will work from their home and will also be required to go into the field.

Field visits could include home visits, visiting member in group home/hospital setting, or attending community/agency meetings.

Position Purpose:

Provides resource support to members and connects members to local resources for various services (e.g., employment, housing, independent living, justice, foster care) based on care/service assessment and plans. Works with leadership to identify the best, most appropriate local resources for member's needs based on industry best practices.

  • This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs.
  • The caseload is 100 members.
  • This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.
  • Field visits could include home visits, visiting member in group home/hospital setting, or attending community/agency meetings.



Performance expectations:

  • Compliance with care management policies/procedures including outreach and documentation expectations.
  • Individuals will be audited on a monthly basis to ensure charts are in compliance.



Candidate Requirements
Education/Certification Required: Bachelor's Degree Preferred: n/a
Licensure Required: Drivers License Preferred: n/a
Years of experience required:

2 years of pediatric case management experience

Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.

Microsoft Office- Outlook, Word, and Excel

Additional qualities to look for:

Health plan experience

Foster care and/or adoption experience


  • Top 3 must-have hard skills stack-ranked by importance


1 2 years of pediatric case management experience
2 Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.
3 Health plan experience
