MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor's Degree in Nursing (BSN) or Registered Nurse actively pursuing a Bachelor's Degree, with the exception of those already working in the position.

2. Current WV State licensure as a Registered Professional Nurse.

EXPERIENCE:

1. Minimum of 3 - 5 years clinical experience required with demonstrated competence.

CORE DUTIES AND RESPONSIBILITIES:The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an all-inclusive list of all responsibilities and duties. Other duties may be assigned.

1. Facilitate suitable and timely bed placement for patients by problem solving with appropriate departments, administrative personnel, and Bed Placement/Admission Coordinator.

2. Oversees all patient care activity on their assigned Patient Care Tower during designated shift and plans for anticipated activity for other shifts. Communicates bed availability and patient placement prioritization plan in conjunction with the Bed Placement/Admission Coordinator. Collaborates with the other Patient Care Tower House Supervisor in maintaining bed flow and patient care activities. Acts as a resource and facilitator to nursing and any admitting unit to expedite the bed assignment process.

3. Round on units regularly to expedite admissions throughout the day as appropriate and to ensure optimal patient care. Ensures Hourly Rounding is being performed on off-shifts and weekends per Nursing Division guidelines.

4. Attend/facilitate Bed Huddles to identify and organize patient placement plan.

5. Serve as a central problem solver for staffing office, admitting, attending physicians, nurse managers, charge nurses, team leaders, and front line staff for all patient care, staffing and patient placement concerns.

6. Monitor occupancy levels and consult Bed Placement/Admission Coordinator and Nurse Administrator to evaluate the initiation of the High Census Plan.

7. Communicate regularly with Bed Placement/Admission Coordinator and Nurse Administrators in regards with issues with the potential to impact staffing or patient care.

8. Facilitate bed assignment process in conjunction with Bed Placement/Admissions Coordinator to decrease wait time.

9. Monitors Unit Bed Boards and discharges and notifies the Bed Placement/Admissions Coordinator to discharge patients from the computer system in a timely manner as appropriate.

10. Participate in the interpretation, development, implementation, and ongoing evaluation of policies and procedures related to nursing care.

11. Recommend and facilitate improvements in the patient care process.

12. Management decisions reflect integration of the West Virginia Nurse Virginia Nurse Practice Act, Collaborative Practice Model, the Joint Commission standards and other relevant standards of care/practice.

13. Demonstrates appropriate patient/family/self-safety practices.

a. Maintain certification in ACLS, PALS, or ENPC, TNCC, ASLS.

b. Wear and utilize the designated communication device for appropriate communication between coworkers and other members of the healthcare team. Maintains constant communication availability throughout assigned shift.

c. Documentation and signatures are legible, concise and complete.

d. Demonstrates knowledge on use of evacuation and other safety related equipment.

14. Responds and leads all Rapid Response and Code responses.

a. Functions as a clinical resource to nursing staff during resuscitation events.

b. Functions as clinical resource to nursing staff when the need for Rapid Response Team (RRT) intervention is required.

c. Functions as clinical resource to improve patient flow.

d. Functions as clinical resource in provisions of nursing care.

e. Functions as staff education resource.

f. Facilitates performance improvement activities as part of routine job activities.

g. Demonstrates knowledge of CCMC Emergency Management Plan including the following codes:

1. Code Blue (Cardiac Arrest)

2. Code 1 (Assault)

3. Code Utility (Utility failure)

4. Code Red (Fire)

5. Code Weather (Inclement weather)

6. Code Gray (Hostage Situation)

7. Code Amber & Code Walker (Missing Person)

8. Code Orange (Hazmat)

9. Code Black (Bomb Threat)

15. Provides direct patient care in situations awaiting the arrival of an on-call nurse or other extraordinary situations. Facilitates transfers of Critical Care patients from the Peri-Operative area or inpatient unit to a Critical Care area and provides proper report off to accepting unit.

16. Oversees, advises, participates and/or finds appropriate resources for patient care activity as identified per shift to facilitate safe care.

17. In conjunction with the clinical manager, oversees the coordination and the allocation of available resources, in assigning nursing staff according to established staffing and scheduling practices to meet patient care hour requirements.

18. Recognizes the need for and supports performance management (PM), facilitates implementation of PM findings on unit, and actively participates in PM activities.

19. Recognizes poor work performance and noncompliance to hospital policies and procedures, conducts management/staff conferences, assists staff with performance improvement and compliance per Human Resources procedures and provides accurate documentation of such interactions.

3. Ability to organize and prioritize time and tasks to achieve a well-coordinated work effort and to efficiently meet work schedules including an ability to integrate multiple factors which may have an impact on patient care, including variance in human resources.
