With guidance and direction from the Director of Nursing and Director of Homeless Services, the Homeless Program RN Case Manager is responsible for providing compassionate and comprehensive case management services to individuals experiencing homelessness. This role involves conducting health assessments, developing personalized care and housing plans, and advocating for clients' access to housing and healthcare resources. The RN Case Manager works collaboratively with community organizations and service providers to navigate complex systems, ensuring that clients receive the support they need to achieve and maintain stable housing and overall well-being. Through ongoing case management and advocacy, this position helps empower vulnerable populations to overcome barriers and improve their quality of life overcome barriers and improve their quality of life.
