Job Summary
The Case Manager will work independently but receives direction concerning the overall goals and objectives of the department and participates in actions to achieve these departmental goals. Performance of case management and care coordination is facilitated primarily through telephonic and electronic and/ or on-site communication with UCDH personnel and individual patients on a longitudinal, scheduled, or as needed frequency. This role has responsibility for the assessment of patients of infant, children, adolescent, adult, and geriatric age groups. - Valid California Registered Nurse License without restriction
- Graduation from an accredited school of Nursing RN program.
- Two years' experience with case management of patients chronic and complex conditions
- Two years clinical, hospital-based experience, inpatient or outpatient setting.
- Experience in identifying and analyzing care/service needs for patients.
- Knowledge of nursing care plans and ability to develop care plans utilizing evidence-based guidelines.
- Skills to perform in-depth chart analysis to provide accurate information in a concise, informative manner.
- Ability to perform work in a telephonic and/ or on-site setting, performing assessments, disease management, self-management education, and complex case management of high-risk patients.
- Knowledge of care/services provided in differing levels of care, i.e., ambulatory, and institutional settings, community-based resources, skilled nursing facilities, home health, hospice, and palliative care.
- Analytical skills to identify and obtain pertinent data from multiple sources to identify patient care needs.
- Ability to make independent judgments based on clinical knowledge and experience required in planning for short, immediate, and long-term goals.
- Ability to work with all levels of staff, both inter- and intra-departmental to create a cooperative relationship and positive work environment.
- Effective time management skills to accommodate special projects, deadlines, and priorities.

Work Environment
