Responsible for completing intake interviews; determining need for medical and psycho-social evaluations; reviewing therapist and physician evaluations, care objectives, and plans; coordinating services being provided; advocating for needed services and entitlements.
Coordinating services being provided; arranging resources; observing and evaluating treatments and responses; advocating for needed services and entitlements; conducting interdisciplinary meetings and evaluations and assisting manager with the day-to-day operations of the department. Does this position require Patient Care? Yes Essential Functions -Determines approach by evaluating the physical, psychological, functional, environmental, and financial needs of patients. Determines psychosocial needs and the compound medical needs of patients. -Defines care goals of patients by providing education, information, and direction to each individual and family -Audit case file on a regular basis for completeness and accuracy. -Assist in discharge planning as required -Participates in case staffing and reports concerns -Attends mandatory staff meetings and training sessions.
