Responsible for providing administrative support to the case management team, facilitating effective patient care coordination, and ensuring efficient workflow within the department. Essential Functions
-Assist case managers with the coordination of patient care services, including scheduling appointments and follow-ups.

-Maintain accurate and organized patient records, ensuring compliance with confidentiality and regulatory standards.

-Prepare and compile necessary documentation for case reviews and meetings.

-Communicate with patients and their families to provide information on services, resources, and discharge planning.

-Collect and input data into electronic health record (EHR) systems to support case management processes.

Education
High School Diploma or Equivalent required and Associate's Degree Healthcare Administration preferred or Associate's Degree Related Field of Study preferred

Experience
Experience in a healthcare administration, clinical office operations or related role 2-3 years required and Experience in case management 0-1 year preferred
Knowledge, Skills and Abilities
- Strong organizational skills and attention to detail.
- Excellent verbal and written communication abilities.
- Proficiency in using electronic health record (EHR) systems and Microsoft Office Suite.
- Ability to work effectively in a fast-paced environment and manage multiple tasks.
- Knowledge of medical terminology and healthcare regulations.
