Job Details

The social worker in the Cancer Support & Welcome Center will help develop, evaluate and implement psychosocial support programs and educational groups both virtually and in person. The Social Worker assesses the psychological needs of patients and provides information, supportive counseling, crisis intervention, and community resource linkage to those impacted by cancer. The social worker is involved in daily operations of programs and community connections including the Patient and Family Advisory Council.

Job Description

ESSENTIAL FUNCTIONS:

  • Conducts psychosocial assessments, develops and implements a plan of intervention that identifies the patient's needs relating to crisis intervention, emotional support, counseling and coordination of community resources for patients and families.
  • Collaborates with all members of the healthcare team, patient and family to develop and implement a patient-centered plan of care.
  • Is an active member and contributor to Schwartz Center Rounds and The Patient and Family Advisory Council
  • Obtains, interprets and communicates necessary information regarding patients and families including social history information to staff and physicians.
  • Provides written assessment to patient records and insurance carriers.Interacts with co-workers, visitors, and other staff consistent with the core values of the Hospital
  • Interacts with co-workers, visitors, and other staff consistent with the iSCORE values of Jefferson.
  • Assist with recruitment for psychosocial research conducted at SKCC including assisting with database management
  • Provides individual and family supportive counseling to patients with cancer or at risk for cancer to assist patients and families in coping and accessing services to evaluate and treat a cancer diagnosis.
  • Provides support and interventions for staff distress

OTHER FUNCTIONS AND COMPETENCIES:

  • Conducts psychosocial assessments, develops and implements a plan of intervention that addresses identified needs related to emotional support, counseling services and/or coordination of needed community resources for patients and families.
  • Collaborates with all members of the healthcare team, patient and family to develop and implement a patient centered plan of care. Obtains, interprets and communicates necessary information regarding patients and families, including social history information, to staff and physicians.
  • Refers patient to hospital and community based services to meet patient continuum of care needs. Such interventions may include referral for financial, psychiatric, rehabilitative, outpatient hospice, drug and alcohol treatment, support groups, transportation.
  • Works with the Director of Clinical Trials to enhance patient enrollment in clinical trials.
  • Provides a link between the Jefferson Infusion Center, the Kimmel Cancer Center and community support agencies serving the needs of cancer patients.
  • Provides written assessment for patient records
  • Meets with the patients in the Jefferson Infusion Center to inform them of available education and support programs, both in the Kimmel Cancer Center and community at large.
  • Provides advocacy services to ensure linkage to appropriate services or resources for patients with no insurance, drug and ETOH addictions, mental health needs, homelessness, complex clinical diagnoses.
  • Provides ongoing emotional support and counseling services to patients/families to help them cope with biopsychosocial problems related to patient's illness such as adjustment to dx, adjustment to acute/chronic/terminal illness, changing family roles, body image changes, financial concerns, insurance issues, advanced directives.
  • Documents in accordance with hospital and departmental standards.
  • Provides direct services for guardianship, power of attorney, health care surrogate, and advanced directives
  • Participates in multidisciplinary healthcare team meetings.
  • Initiates and facilitates referrals through the Cancer Support and Welcome Center.
  • Provides individual and family supportive counseling and case management to patients with cancer or at risk for cancer to assist patients and families in coping and accessing services to evaluate and treat a cancer diagnosis.
  • Participates in development, implementation, evaluation, and revision of clinical protocols/guidelines in the Cancer Support and Welcome Center

EDUCATIONAL/TRAINING REQUIREMENTS:

  • Masters in Social Work required.

CERTIFICATES, LICENSES, AND REGISTRATION:

  • Social Worker State Licensure in the Commonwealth of Pennsylvania required

EXPERIENCE REQUIREMENTS:

  • Healthcare experience or post MSW healthcare experience.
  • 1-3 Years of Healthcare experience preferred.
  • Research Experience strongly desired
