The Sr Medical Outreach Assistant serves as a liaison between the patient/family and the health systems providing navigation to facilitate access to care, resources and improved communication to assure patient follow-up, long-term chronic care management and reduce health needs. Experience REQUIRED:
Minimum of three (3) years' experience working in a health and/or community health setting
Previous experience utilizing strong interpersonal, problem solving and analytical skills
Excellent customer service and conflict resolution skills including the ability to handle/escalate difficult situations
Experience PREFERRED:
Five (5) years' experience working in community service, health or social service sector
Experience working with underserved populations
Education/training REQUIRED:
High School Diploma or equivalent
Education/training PREFERRED:
Associate or Bachelor's Degree in Health Sciences or Social Services from an accredited program
Independent action(s) required:
Sets goals and plans to accomplish role/responsibilities.
Able to handle multiple priorities and manage time to meet workload goals.
Documents interactions with clients in appropriate format and software.
Supervisory responsibilities (if applicable):
Provides assistance with supervisory responsibilities, including performance evaluations, onboarding and training.
Additional position requirements:
May work all three shifts, weekends and holidays. Individual shift rotation and schedules will be established with the Manager who will determine the needs for appropriate coverage of services.
Schedules, including requirements of shift rotations and hours of work, including weekends and holidays, may be adjusted as necessary to meet program and work requirements and/or as needed.
Outreach services may be conducted by face-to-face contacts during home or community visits, telephone contact and/or written communication.

Physical: Lifting 20-50 lbs.
Activities: Prolonged standing, Prolonged sitting, Walking (distance), Reaching (overhead, extensive, repetitive)
Mental/Sensory: Strong recall, Reasoning, Problem solving, Hearing, Speak clearly, Write legibly, Reading, Logical thinking
Emotional: Fast pace environment, Able to handle multiple priorities, Frequent and intense customer interactions, Able to adapt to frequent, change

Join the nationally recognized You CAN Control Asthma Now (UCAN) program at the Children's Hospital of Richmond at VCU. As the Sr. Medical Outreach Assistant, you will be integral to helping our patient's families learn about asthma, and to address issues that can get in the way of well controlled asthma. You will spend most of your time in the community meeting with families at their homes in neighborhoods in and around Richmond, providing education at health fairs, and reaching out to families by phone. You will need a working and reliable personal vehicle. Milage reimbursement is provided. You will be part of a dynamic multi-disciplinary team who have a passion for treating asthma and the families with whom we work. You should be comfortable working with individuals of all colors, backgrounds, and socio-economic statuses. You will be spending time in low-wealth communities so previous experience in these neighborhoods is a plus.
