Job Summary and Responsibilities

Acentra Health is looking for a Waiver Services Assessor to join our growing team.

Job Summary:

The Waiver Services Assessor is responsible for conducting annual face-to-face assessments for Medicaid Fee-for-Service (FFS) members residing in the community who are receiving Long-Term Services and Supports (LTSS) under an approved waiver program. Using the Level of Care Evaluation Review Instrument (LOCERI), the assessor evaluates whether members continue to meet Virginia Department of Medical Assistance Services (DMAS) criteria for waiver eligibility.

Responsibilities:

  • Conduct in-person assessments in member homes or community settings using the LOCERI assessment tool
  • Evaluate the member's need for continued personal care and private duty nursing services
  • Assess the member's status across three required domains:
    • Functional Capacity
    • Medical or Nursing Needs
    • Risk of Institutionalization in the absence of waiver services
  • Gather information through observation, interviews, and limited physical demonstrations (no hands-on care provided)
  • Ensure documentation is thorough, accurate, and submitted within required timelines
  • Maintain a working knowledge of state waiver eligibility requirements and DMAS policies
  • Communicate effectively with members, caregivers, and interdisciplinary care teams
  • Escalate concerns or changes in condition that may impact member safety or eligibility
  • Participate in ongoing training and quality reviews related to the LOCERI tool and assessment protocols
  • Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules

The list of accountabilities is not intended to be all-inclusive and may be expanded to include other education- and experience-related duties that management may deem necessary from time to time.


Qualifications

Required Qualifications

  • Current, unrestricted license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Virginia
  • Must reside within the northern or central regions of Virginia (i.e. Alexandria, Fairfax, Page, Arlington, Henrico counties)
  • Minimum of two years' experience in a home care setting, home health, or case management
  • Reliable transportation and a clean motor vehicle record (MVR) for field-based travel to member's homes
  • Ability to work independently in the field
  • Reliable home internet connection
  • Strong interpersonal and communication skills
  • Proficiency with documentation systems and electronic health records

Preferred Qualifications

  • Experience working with Medicaid populations
  • Experience with utilization management
  • Experience with care coordination
  • Experience with management of chronic care population
