Essential Job Duties |
Primary Duties and Responsibilities of the RN Case Manager/Triage:
- The RN Case Manager works directly with patients, assessing need and connecting patients with resources, services and outside medical professionals as indicated.
- Works collaboratively with the Health Information Management (HIM) team, which oversees patient access and referral coordination, to resolve patient-related concerns and issues. Triage patients who present requesting appointments for Primary Care or GYN Services.
- Performs assessment to determine priority and disposition of patients and documenting in the electronic health record.
- Receives documentation from the CH After Hours Triage system and collaborates with appropriate CH leaders and/or staff to ensure follow up.
- Serves as a liaison between the NC State University CARES Team and NC State University Campus Health Team.
- Helps to identify patterns in the delivery of care of the Campus Health team and will provide feedback to guide a portion of the Campus Health Quality Improvement activities.

Qualifications
Minimum Experience/Education |
Licensed to practice as a Registered Nurse in the state of North Carolina. |
Other Required Qualifications |
- Previous RN triage experience
- Experience with Electronic Records software
- Strong verbal and written skills
- Problem-solving skills
- Multitasking skills
- Prioritization skills
- Experience with triage nursing protocols and following standing orders.
- Must Report to work for Adverse Weather and related scenarios and for public health emergencies

Preferred Qualifications |
- Previous case management experience.

Required License or Certification |
This position requires an unencumbered license to practice nursing in the state of North Carolina, CPR- BLS and ANCC Nursing Case Management Certification (CMGT-BC) or equivalent, within two years of being hired. |
