Washington County Behavioral Health Division within the Health and Human Services (HHS) Department is recruiting for a
Youth and Young Adult Intensive Care Coordinator
(ICC)! This position will primarily serve youth and young adults with complex behavioral health needs, with the primary goal of improving health outcomes through collaboration with the individual, system providers and partners, and other natural supports. About the Opportunity:
The Intensive Care Coordinator (ICC) will work to ensure Oregon Health Plan (OHP) members residing in Washington County have access to care coordination regardless of eligibility for specific services. In this position, you will work with youth and transitional-aged young adult clients to provide needs assessment, care planning, connection to service delivery systems, and coordination with behavioral health, physical health and dental providers. Using a person-centered approach, you will schedule and facilitate interdisciplinary team meetings and coordinate with partners including legal systems, Department of Human Services (DHS), Aging and People with Disabilities (APD), and Developmental Disability services with the goal of improving communication and health outcomes. You will also maintain written documentation of all contact with the clients, their family, and system and service providers.
This position may be eligible for a hybrid work schedule.

- Experience and passion for working with youth and young adults with complex behavioral health needs.
- Understanding of the needs of Black, Indigenous, and People of Color (BIPOC) communities in Washington County.
- Exceptional organizational skills and ability to prioritize and manage a variety of tasks in a fast-paced environment.
- Ability to complete accurate documentation in a timely manner.
- Effective collaboration skills and ability to build and maintain relationships with clients and system partners.
- Understanding of Medicaid systems in Oregon.
The following are preferred qualifications, but not required:
- Bilingual/multi-lingual
- Certified Alcohol and Drug Counselor (CADC) credential
- Experience working with clients with dual diagnosis (behavioral health and substance use disorder)

MINIMUM QUALIFICATIONS
-
Education and Experience:
- A Master's degree in Counseling, Psychology, Social Work, or related field; AND three (3) years of increasingly responsible experience in providing case management or services coordination in a human services related field; AND
- Must be certified by the Mental Health and Addictions Certification Board of Oregon as a Qualified Mental Health Professional or hold an approved clinical license and meet the criteria of a Qualified Mental Health Professional as defined in the Oregon Administrative Rules.
