The Transition Specialist is responsible for conducting an educational assessment and developing and implementing an individualized tutorial plan based on the individuals' learning needs and abilities.

Key Responsibilities include:

  • Transition Specialist creates a safe environment for clients, provides direct supervision of clients, empowers clients, and teaches new tools for coping to clients. The Transition Specialist will participate as an integral member of the treatment team and in developing the continuing care plan with regard to educational placement upon discharge. The Transition Specialist will provide age and developmentally appropriate education in a manner consistent with the philosophy and objectives of Compass Intervention Center.

Qualifications

Position Requirements:

  • Minimum of 5years teaching experience.
  • Provide transition assessments (interest and career inventories, aptitudes, work skills) to allstudents (i.e. Aviator, Careerscope, or other research based, assessments, etc)
  • Ability to maintain a "Transition Profile" for each student in their folder that tracks assessments and their results with a suggested timeline of implementation
  • Ability to collaborate with community businesses to establish work experience and shadowing opportunities
  • Establish and maintain a transition assessment and services resource file to be utilized
  • Collaborate with IEP teams to write meaningful transition plans, determine post-secondary goals and transition services, and/or serve as a resource to special education case managers
  • Organize a "Transition Document" for students and their families that would provide information from workplaces, colleges, transition program, community resources, etc.
  • Teach lessons and/or provide activities to study hall/ study skills students on a regular basis re: transition topics
  • Life Skills
  • Prefer knowledge of effective teaching skills and educational strategies with a special education population, individual and group management skills, therapeutic relationships and processes, age specific growth and development; limit setting and crisis and behavior management.
