Job Summary and Responsibilities

Acentra Health is looking for a RN Clinical Assessor - I/DD Waiver to join our growing team.

Job Summary:

Join our team as an RN Clinical Assessor and be part of the transformative West Virginia I/DD Waiver Program. This Medicaid home and community-based services (HCBS) imitative empowers individuals with intellectual and developmental disabilities to thrive within their communities while receiving personalized support.

As an RN Clinical Assessor within the I/DD Waiver Program's Intensively Supported Settings and Licensed Group Homes (ISS/GH), you'll play a crucial role in ensuring members receive 24/7 services tailored to their needs. These services are designed based on individualized person-centered plans, providing a comprehensive approach to support.

Your responsibilities will include conducting reviews in collaboration with WV I/DD Waiver provider agencies, focusing on compliance with HCBS settings rules, policy requirements, and the overall well-being pf program participants. You'll have the unique opportunity to visit members in their homes, observe living conditions, interview members about their satisfaction, collect vital information from staff regarding HCBS services and supports, and review associated documentation to identify areas of compliance or concern.

Your goal is to guarantee that program participants enjoy the full benefits of community living, with access to services in the most integrated and least restrictive settings possible. You'll champion their rights, freedoms, and dignity to the maximum extent by performing onsite reviews at provider agency offices and member's residents throughout the state.

Responsibilities:

  • Conduct comprehensive onsite health and safety assessments for I/DD Waiver members residing in Intensively Supported Settings/Group Hoemes (ISS/GH)
  • Thoroughly review and analyze various documentation, including treatment plans, medication records, incident reports, care recommendations, training certifications, and associated records
  • Conduct interviews and observations involving program members, direct care staff, and professional staff
  • Identify and meticulously document specific quality indicators to monitor and track program performance
  • Collect, assess, and report review-related data, ensuring compliance with policy and safety indicators
  • Summarize findings from reviews, recognize trends, and prepare comprehensive reports
  • Provide education and guidance on compliance and issue remediation
  • Attend training sessions and scheduled meetings, and maintain up-to-date information for conducting reviews
  • Travel to homes and facilities within the state of West Virginia is required, and occasional overnight stays may be necessary
  • Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules

Qualifications

Required qualifications:

  • West Virginia residence
  • Reliable transportation and a clean motor vehicle record (MVR) to be able to conduct field work involved
  • Hold a current, unrestricted Registered Nurse (RN) license for the State of West Virginia
  • Minimum of two years of medical experience
  • Proficiency with computer navigation including internet and Microsoft Office products i.e. Teams, Outlook, Excel

Preferred qualifications:

  • Two years of home health experience
  • Bachelor's degree
  • Experience with the West Virginia I/DD Waiver program
  • Experience in working with individuals with intellectual and/or developmental disabilities
  • Prior exposure to Medicaid/Public Sector services
  • Familiarity with the WV I/DD Waiver CareConnection or Atrezzo
  • Knowledge of WV I/DD Waiver program policies, procedures, and other available services for program members
  • Understanding of Medicaid-funded home and community-based services (HCBS) standards
  • Effective interaction with healthcare professionals, direct care staff, program participants, families, and support networks
