Reporting to the Manager, Quality Improvement, the Senior Program Manager, Health Equity & Quality Improvement is responsible for Health Equity & Quality Improvement (QI) programs. You will oversight and serve to connect health equity to QI and care experience. You will work on the QI team, the Sr. PM, HEQI will collaborate with other QI & Population Health Management (QPHM) staff to ensure SFHP and provider alignment on health equity priorities. SFHP is proud to offer a hybrid work environment. As such, this role will be required to be onsite, in office a minimum of 4 days per month. Salary: $110,000 - $140,000 per year WHAT YOU WILL DO:
- Plan health equity planning and goal setting, regulatory and accreditation reporting, including the Health Equity Quality Measure Set, and leading SFHP's health equity efforts, programs and interventions.
- Coordinate major Health Equity & QI priorities by supporting measure identification and prioritization based on regulatory requirements and NCQA Health Plan and Health Equity standards.
- Lead Health Equity organizational projects including health equity strategic planning, collection of member information to improve culturally and linguistically appropriate services and supporting SFHP's roadmap to NCQA Health Equity Accreditation.
- Collaborate with the Chief Health Equity Officer (CHEO) and Quality & Population Health Management (QPHM) staff in producing health equity analyses, planning and implementing health equity intervention as part of the annual Quality Improvement & Health Equity Transformation Program Description, Work Plan and Evaluation.
- Work with the Quality Data Analytics team to ensure stratified reporting of Health Equity Quality Measure Set and project manages required improvement projects or participates in projects that result from measures not meeting set benchmarks.
- Produce QI and HE related materials to solicit feedback from SFHO's external groups including the Quality Improvement and Health Equity Committee, the Member Advisory Committee, and provider network collaborative meetings.
- Provide guidance in project teams related to Quality and Health Equity, to ensure that project work is measurable, impactful, cost-effective, and consistent with organizational goals.
- Participate in QI & Health Equity work plan development, completion of work, and mid- and long-term strategic planning.
- Identify root causes for QI & Health Equity through measurement, collaboration, and system analysis.
- Be the coach to diverse clinic staff to facilitate ongoing improvements.
- Model leadership in quality improvement (i.e. PDSA model).
- Complete other Health Equity related tasks as assigned by the CHEO.
WHAT YOU WILL BRING:
- Required: Bachelor's degree in healthcare administration or a related field or equivalent work experience.
- At least five (5+) years' experience in program management in healthcare operations or quality improvement or equivalent work experience.
- Prior experience leading programs.
- Master's degree in healthcare field; experience with health equity, public health, or quality within a managed care or health care delivery system setting; Project Management Certification is helpful
- Familiarity with the Model for Improvement, healthcare, managed care terminology, Medi-Cal. is helpful.
