Sentara RMH Medical Center is now hiring an Inpatient Case Manager who will serve emergency department patients. This is a full-time, evening shift position.

Requirements:

Nursing degree and RN License

1 year of Acute Care Experience

1 year of Nursing Experience

1 year of Case Management experience

Duties/Responsibilities

  • Responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, primary focus on the patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS.
  • Directly support those in-hospital patients identified in need of comprehensive case management.
  • Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions.
  • Develops and/or implements a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver.

Job Summary

Develops and/or implements a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only.

BLS within 90 days of hire.
BSN is preferred. All Registered Nurses who do not have their BSN will be required to sign a BSN Agreement committing to successfully obtaining their BSN within 5 years of hire.

Qualifications:

N-2YR - RN-Associate's Degree (Required), N-3YR - RN-Diploma (Non-degree) (Required), N-4YR - RN-Bachelor's Level Degree (Required), N-6YR - RN-Master's Level Degree (Required) Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI, Registered Nurse (RN) Multi State - Nursing License - North Carolina, Registered Nurse (RN) Multi State - Nursing License - Virginia Department of Health Professionals (VADHP) Acute Care, Case Management, Nursing
