Overview

The Care Facilitator Supervisor works closely with the Director of Patient Care Coordination & Access to oversee a team of care facilitators who support primary care providers servicing home and community-based patients, primarily in long-term care (LTC) and assisted living settings. This role ensures that care facilitators effectively manage administrative and operational tasks, including scheduling patient visits, answering phones, obtaining, and organizing medical records, coordinating with providers and facilities, and delivering excellent customer service. The supervisor fosters a culture of teamwork, accountability, and continuous improvement while demonstrating strong communication, critical thinking, and problem-solving skills to support both staff and patient care operations.



Responsibilities

    Builds rapport and trust with the team.
  • Provides oversight and direction to the care facilitators to ensure daily operational support.
  • Responsible for hiring, coaching, developing and evaluating employees.
  • Assists in the completion of departmental audits, reports and QA.
  • Responsible for ensuring daily staffing coverage is adequate.
  • Monitors and evaluates effectiveness of patient care and the processes for achieving positive patient outcomes.
  • Assists in the development and implementation of standardized workflows and processes to increase productivity, efficiency, and outcomes.
  • Manages service recovery efforts with patients, families, facilities, and providers, as appropriate.
  • Completes special projects and assignments.
  • Adheres to standards of the Company's Code of Conduct, policies, Health Insurance Portability and Accountability Act and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.
  • Attends, participates in and/or conducts internal staff development programs, obtains continuing education as required by Company policy and regulations. Maintains and enhances clinical practice skills.
  • Responsible for managing and completing payroll for team.


Qualifications

  • High school diploma/ GED required.
  • College degree preferred.
  • Prefer 3+ years of geriatric clinical experience.
  • Prefer 2+ years of management experience.
  • Demonstrates professional interpersonal, verbal, and written communication and active listening skills.
  • Strong assessment and problem solving skills.
  • Ability to inspire and motivate others.
  • Effectively manages time and priorities.
  • Knowledge of compliance (including clinical records) of all local, state and federal laws regarding licensure and certification with Home Health Care compliance standards.
