Position Title:RN Care Manager-Complex Care Ambulatory
Department:Transitional Care Management/P4P
Job Description:
This RN would be responsible for coordinating and delivering patient-centered, comprehensive care management services for individuals with heart failure and other high-risk conditions. This role will focus on reducing hospital readmissions, improving clinical outcomes, supporting self-management, and enhancing quality of life through evidence-based interventions and interdisciplinary collaboration. Prefer: 3-years' experience in cardiology, heart failure, and/or complex care management with a strong knowledge of evidence-based heart failure management, transitions of care General Description: As a member of a multidisciplinary care management team, the nursing case manager is accountable and responsible for coordinating the care and services of selected patient populations. General Responsibilities:
Minimum Qualifications: Education: Graduate from an accredited school of nursing required. Bachelor of Science (or higher) in Nursing Board Approved Program preferred. Experience: (3) years clinical nursing experience in an acute care facility. License(s)/Certification(s)/Registration(s) Required: Current RN License issued by the Oklahoma State Board of Nursing, or a current multistate Compact RN License (eNLC). Current Basic Life Support issued by the American Heart Association required upon hire. Knowledge, Skills and Abilities:
