POSITION SUMMARY
CLUES' Certified Community Behavioral Health Clinic (CCBHC), Abriendo Caminos, will be providing immediate access to supportive, high-quality, culturally-responsive and integrated Behavioral Health services and pathways to wellness for the Latinx Community, regardless of insurance status or ability to pay. The CCBHC model provides CLUES with a model for sustainable Behavioral Health service provision, which translates to increased access to consistent care for wellness in the Latinx community.

The Behavioral Health Intake and Care Coordinator provides intake support to the Behavioral Health department, which includes Mental Health, Mental Illness & Substance Use Disorder (MI/SUD), CTSS, and other services. The Behavioral Health Intake and Care Coordinator conducts a comprehensive intake process with individuals requesting services and follows up with clients and referral sources to coordinate care and ensure continuity of services to clients. This person assists the Behavioral Health Intake & Care Coordination Supervisor with administrative duties related to the intake process, including but not limited to verifying client eligibility and/or insurance and maintaining client and program data.

The care coordination part of this role is to ensure that individuals requesting services are able to access any appropriate health-related services and to ensure the continuity of care by collaborating with providers involved to ensure positive outcomes. This position will be part of a team of Intake and Care Coordinators who will be cross-trained in intake and care coordination duties and will support each other in providing access and continuity of care for participants.

Schedule: Full-Time. Generally, Monday - Friday office hours, with occasional evening hours to accommodate clients as needed.

Estimated Pay: $21.25 to $25.00 (depending on experience & qualifications).

ESSENTIAL DUTIES AND RESPONSIBILITIES

Client Intake and Care Coordination

  • Schedule intake appointments for clients.
  • Conduct phone and/or in-person intake interviews in Spanish and English for client referrals in an engaging, respectful and efficient manner in order to obtain all the information included in the pertinent forms such as demographics, personal behavioral health and chemical health history, and insurance information.
  • Collect health-related data relevant for outcomes data reporting to various funders.
  • Explain rules of confidentiality, client rights, program rules, and intake forms to clients.
  • Conduct risk assessments for the threat of self-harm or harm to others, child abuse, and elder abuse. Report findings of risk assessments to appropriate authorities, if deemed necessary. Training will be provided by clinical supervisor on risk assessment.
  • Maintain up-to-date and accurate records for all referrals and enter intake information into Credible (the electronic health record) or other database(s) used by CLUES.
  • After conducting the intake interview, assign clients to staff in a timely fashion.
  • Follow up with clients once assigned to staff in order to coordinate continuity of services (i.e., scheduling first appointment).
  • Help clients connect with other agency services and facilitate internal referrals. Follow up with referral sources (case managers, doctors, CLUES staff, etc.) regarding status of referrals.
  • Prepare and present reports with actual, up-to-date information regarding the intake process.
  • Provide continuity of care by following participants during their episode of care to ensure that they access relevant services within CLUES and outside CLUES.


Client Eligibility and/or Insurance Verification

  • For clients with insurance, check insurance benefits and call insurances to get authorization approvals.
  • For clients who have been approved to receive county funding to cover treatment costs (outside referrals), verify approval of client by obtaining DHS authorization form (with DHS authorization #). Call the county to verify funding if DHS authorization form has not already been received
  • When necessary, contact county to check if client completed an assessment outside CLUES before they can start treatment and CLUES will be able to bill.


General Duties

  • Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations, and ethical standards.
  • Promote the mission, operating values, and agency services of CLUES within the community.
  • Carry out all other duties as assigned by supervisor.

QUALIFICATION REQUIREMENTS
Education, Experience & Key Abilities

  • High school diploma or GED required. Bachelor's degree in behavioral health, public health, or related field preferred.
  • Bilingual in English and Spanish and cultural responsiveness and awareness required.
  • Office administrative experience highly preferred.
  • Experience working in healthcare setting, especially in chemical dependency or mental health fields, required.
  • Experience in third party billing and/or medical coding preferred.
  • Additional Requirements per Rule 245G for professional with direct client contact:
  • Must be at least 18 years of age.
  • At the time of hiring, must be free of problematic substance use for at least the two years immediately preceding hiring and must sign a statement attesting to that fact.
  • Must have excellent customer service and interpersonal skills for handling sensitive situations with clients. Must have professional phone etiquette for speaking with clients.
  • Must be well-organized, pay attention to detail, and be able to multi-task in a fast-paced environment.
  • Must have a deep understanding of Latinx cultures, values, and traditions, and demonstrate cultural responsiveness in every aspect of the role, including communication with colleagues, staff, clients, community members; creating forms and documents; and in supervision.
  • Must be able to sign, recertify annually, and continually abide by CLUES Chemical Health & Prevention Programs Chemical Free Attestation and remain in compliance with CLUES Continuing Freedom from Substance Use Problems Policy.
  • Must willingly complete all the required department orientation processes and trainings per timelines assigned, including on-going trainings.


Language Skills

  • Effective communication in English and Spanish, both verbally and in writing, required.
  • Ability to read, analyze, and interpret written materials, including reports, contracts, and proposals.
  • Ability to effectively present ideas and information in an organized and effective manner, including writing reports.
  • Ability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management and external parties.


Computer Skills

  • Proficient and strong working knowledge of email software, the Internet, databases, and common Windows-based programs, including Microsoft Office Word, Excel and PowerPoint required.
  • Must be willing to attend training to advance technical skills and maintain appropriate skill levels to perform job duties.




WORKING CONDITIONS

Background check

  • Must pass a criminal background check, DHS background study, and MS 604.20 background check. As a condition of continued employment, staff person may be required to submit to periodic re-verification of these background checks.
