POSITION SUMMARY
CLUES' Certified Community Behavioral Health Clinic (CCBHC), Abriendo Caminos, will be providing immediate access to supportive, high-quality, culturally-responsive and integrated Behavioral Health services and pathways to wellness for the Latinx Community, regardless of insurance status or ability to pay. The CCBHC model provides CLUES with a model for sustainable Behavioral Health service provision, which translates to increased access to consistent care for wellness in the Latinx community.
The Behavioral Health Intake and Care Coordinator provides intake support to the Behavioral Health department, which includes Mental Health, Mental Illness & Substance Use Disorder (MI/SUD), CTSS, and other services. The Behavioral Health Intake and Care Coordinator conducts a comprehensive intake process with individuals requesting services and follows up with clients and referral sources to coordinate care and ensure continuity of services to clients. This person assists the Behavioral Health Intake & Care Coordination Supervisor with administrative duties related to the intake process, including but not limited to verifying client eligibility and/or insurance and maintaining client and program data. The care coordination part of this role is to ensure that individuals requesting services are able to access any appropriate health-related services and to ensure the continuity of care by collaborating with providers involved to ensure positive outcomes. This position will be part of a team of Intake and Care Coordinators who will be cross-trained in intake and care coordination duties and will support each other in providing access and continuity of care for participants. Generally, Monday - Friday office hours, with occasional evening hours to accommodate clients as needed. Estimated Pay: $21.25 to $25.00 (depending on experience & qualifications).
