Job Details Community Health WorkerJob Description PRIMARY FUNCTION: The Community Health Worker (CHW) will assist individuals, families, and communities to adopt healthy behaviors; help patients, community members, and their families navigate healthcare systems and insurance plans; and access community and government services. The CHW will provide social support, complete Health Related Social Needs (HRSN) screening, informal counseling, and advocate for individual and community health needs. The CHW will be a liaison between the patient/community members/families, healthcare team, and community-based organizations. Home and community-based visits are work assignment specific but could be an essential function of a CHW's work. Location assignment and work settings could include: outpatient care areas, patient homes, inpatient settings, skilled nursing, community organizations, and participating in community events. Participation in community events and some evening/weekend hours may be required. ESSENTIAL FUNCTIONS:
