OVERVIEW

LightHouse for the Blind and Visually Impaired, headquartered in downtown San Francisco, is looking for a full time People Operations Specialist to support our SF, East Bay, Napa, and North Bay offices and to fill a valued role in our growing organization.



QUALIFICATIONS:

A minimum of a Bachelor's Degree in social work, rehabilitation, nursing, counseling, psychology, or related field. At least year's experience working with client case management in a community-centered setting; with experience working with the disabled, blind, and/or senior community highly desired.

Bicultural and bilingual welcomed, particularly in languages like American Sign Language, Spanish, Cantonese, Mandarin, Russian, Tagalog, or Vietnamese.

Excellent verbal and written communication skills; strong Microsoft office skills and experience in database entry; strong interpersonal skills to relate to staff, blind/visually impaired students, and persons in the community with varied backgrounds and viewpoints. Ability to formulate individual, sequential training plan. Ability to move about within a variety of environmental conditions. Knowledge of braille desired.

Demonstrated attention to detail and in possession of problem-solving skills; flexibility and ability to multi-task; ability to work under pressure in a deadline-driven environment; and grant and project oversight abilities.


JOB RESPONSIBILITIES:


  • Collaborate with Intake Specialists to ensure smooth transition from initial student intakes and assessments to LightHouse PATH plan
  • Collaborate with Intake Specialists to ensure smooth handoff from Intake to Service Navigation and PATH plan
  • Work with department directors and program managers to ensure students have been contacted within ten business days of referral.
  • While students have an open case, check-in monthly with students to ensure PATH Plan meets student needs
  • In collaboration with Senior Director of Curriculum and Connections staff, develop structure to assure accurate and accessible training for students, including their goals and progress.
  • Maintain accurate and timely case notes on each student in Salesforce and Electronic Health Records (EHR) system
  • Work with LightHouse service providers to review and/or amend PATH Plan as necessary
  • Maintain active dialogue with students and their support networks, when appropriate
  • Assist students in signing up for external services and/or make external referrals
  • Collaborate with student and internal/external service providers to meet the student's PATH plan and goals to ensure student's success, or close services if warranted.
  • Be knowledgeable in all LightHouse services and promote service use and improvement across all of LightHouse's service offerings
  • Stay up to date in best practices of disability and aging services
  • Maintain confidentiality of sensitive information and follow LightHouse HIPAA guidelines
  • Ability to follow procedures established by external funding sources and medical service providers
  • Maintain a hybrid work schedule (in-person, 3 days a week), with agreed-upon schedule
  • Travel throughout the Bay Area service area to locations such as LightHouse offices, student residences, community centers, and other agencies where students frequent or receive services.
  • When necessary, accompany students and their support networks to medical appointments, specifically related to vision care.
  • Maintain up-to-date First Aid and CPR certifications, to be paid for by LightHouse


EXPECTATIONS FOR SUCCESS IN THIS ROLE:


  • Ensure entry of all student service data into Salesforce and other relevant systems are entered by Wednesday for the prior week.
  • Submit monthly reporting, billing, and reimbursements accurately and timely by the 5th business day of preceding month.
  • Regularly connect with students and support network as established in Individual Service Plan, with regular connections with each student on case load at least once every 6 weeks.


PHYSICAL REQUIREMENTS:

  • Ability to sit at a desk and perform computer-intensive work for long periods and operate standard office equipment
  • Ability to work at offsite location(s) independently as needed.
  • Ability to travel throughout the Greater Bay Area when required.
