Schedule: Monday - Friday 8:30 AM - 5:00 PM

Requirements


  • Demonstrates knowledge of techniques, procedures, use of equipment and Hospital & departmental policies necessary for job performance.
  • In collaboration with the Case Management Coordinator - RN, uses screening mechanisms to identify patients who are at risk for lengthy inpatient stays, complex clinical issues, high resource use or repeat admissions due to chronic diseases. Intervenes within 24 hours of patient admissions. Documents according to established guidelines
  • Assesses patient & family psychosocial needs, including coping mechanisms, ability to comply effectively with plan of care and desired outcomes. Makes referrals as appropriate. Provides patient & family counseling as appropriate.
  • Works collaboratively with physicians and other health care team members to achieve desired outcomes in accordance with physicians' plans of care. Collaborates as needed with the Utilization Management Committee.
  • Identifies potential or current patient situations which require referral to other members of the health care team such as financial counseling or risk management. Assures discharge plan is implemented and follow-up occurs

  • Serves as a resource for patients, family members, & the community; provides information and referrals as needed.
  • In conjunction with physician, nursing staff & other health care team members, facilitates dischargeplanning process and identifies need for referral to external sources for post-hospitalization needs.
  • Maintains accurate & current knowledge of available community resources including home-health agencies, skilled-nursing facilities, rehab facilities, and other available resources. Explains options to patients & families
  • Maintains accurate & current knowledge of payor practices and regulations that may impact patient outcomes and/or progress.
  • Initiates & maintains all acute-care abstracts for discharge planning/social services
  • Initiates & maintains MedQM data on all patients

Qualifications

MS Social Work from an accredited school of social work.

Must be licensed by the Maryland Board of Social Work Examiners at the LCSW level.

2 years or more of licensed clinical social work experienced in Hospital Health care setting.
