Lymphedema Certified Occupational Therapist OT CLT - Home Health (PRN)

Empath Home Health, a member of Empath Health, is seeking a PRN Home Health Lymphedema Certified OT - Occupational Therapist. The Home Health Lymphedema Certified OT provides occupational therapy services related to patient's level of function and performance of daily living.

Locations: home heath visits available in Fort Myers or Naples area (Lee or Collier counties)

The Home Health Lymphedema Certified OT - Occupational Therapist:

• Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.
• Performs initial and on going assessments to determine level of functioning and patient's ability to perform activities of daily living, including OASIS assessments at appropriate time points.
• Develops and revises a plan of care in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of self-care.

Requirements:

• Bachelor Degree in Occupational Therapy.
• Florida licensure in occupational therapy.
• Minimum of one (1) year experience as a licensed Occupational Therapist.
• CLT - Certified Lymphedema Therapist in the respective state.
• Home health care experience preferred.
