St. Ann's Community is are currently looking for a part time Life Enrichment Care Partner at St. Ann's Home located on the Portland Campus. The primary focus of this position is to assess the recreation and leisure needs of residents, plan and implement interventions, and evaluate the effectiveness of services provided at St. Ann's Home.

Job Description:



  • Completes resident assessment and as a member of the IDT team develops a Comprehensive Care
  • Plan for each individual
  • Assesses the strengths, interests, cultural preferences, limitations as it relates to their recreation and leisure interests of residents, focuses on the resident as the locust of control and supports the resident and/or resident representative in making their own choices and having control over their daily lives
  • Supports the invitation of resident and designated resident representative in to the care plan process and development
  • Develops individualized plan of care with measurable goals integrated into interdisciplinary team approach
  • Plans interventions appropriate to interests, needs and functional level of resident
  • Implements individualized treatment plans that promote maximum resident well-being
  • Observes for behavioral changes and adapts approaches to meet resident needs
  • Evaluates programs and interventions to assure efficiency and effectiveness in meeting residents needs
  • Provides individualized interventions including but not limited to social integration, intellectual, sensory stimulation, creative, physical, religious/spiritual programs and community integration
  • Provides on-going recreation therapy for residents adhering to regulatory and professional standards of Therapeutic Recreation; NCTRC
  • Provide a variety of meaningful programs, as deemed Life Enrichment Care Partner by resident, that emphasize individuals' strengths and abilities, promote physical and psycho-social functioning, offer sensory and intellectual stimulation and promote a sense of usefulness and community spirit
  • Plan and coordinate SAH special events with resident(s) and staff
  • Collaborate in the planning of community wide events and/or intergenerational programming with resident(s) and other staff members
  • Monitor and maintain supplies necessary for program implementation within the recreation budget
  • Coordinate community outings with resident(s) and other staff members
  • Completes resident assessment in Transitional Care as assigned
  • Assesses the strengths, interests, cultural preferences, limitations as it relates to their recreation and leisure interests of residents, focuses on the resident as the locus of control and supports the resident and/or resident representative in making their own choices and having control over their daily lives
  • Maintains timely documentation of MDS, CAAS, assessments and clinical notes
  • Develops individualized plan of care with measurable goals integrated into interdisciplinary team approach
  • Serves as life enrichment ambassador to staff at SAH
  • Empower and support SAH staff to create a culture of Person Centered Care through meaningful engagement
  • Coordinates and supports SAH staff through the Person Centered Care and Annie's Angel grant process
  • Provides in-services and trainings for WCC staff on meaningful engagement and person centered care
  • Maintains timely documentation of events and all required paperwork including but not limited to MDS, CAAS, assessments and clinical notes
  • Documents in clear and concise manner, using appropriate grammar, spelling, legible writing and appropriate use of professional and medical language
  • Maintains timely participation documentation of events and programs in Care Merge
  • Informs residents of resident group meetings including: Resident Council and on-sight groups
  • Supervises volunteers and supports volunteers by providing clear direction and adequate support
  • Conducts resident interviews as needed in various Quality Assurances programs and initiatives
  • Assists in meal rounds in collaboration with IDT team
  • Provides rotating weekend life enrichment coverage
  • Provides rotating evening life enrichment coverage
  • Provides recreation therapy coverage for life enrichment team members as needed
  • Performs tasks as assigned by the Manager


Candidate Requirements:



  • Bachelor's degree (B. A.) from fouryear college or university in the field of Therapeutic Recreation or related area with geriatric experience; Certified Therapeutic Recreation Specialist preferred. CPR certification preferred
