A LONG STANDING HOSPICE AND CARE FACILITY WITHIN THE WOODS CROSS AREA IS LOOKING TO BRING ON AN ADMISSIONS COORDINATOR TO JOIN THEIR TEAM! Schedule: * Rotating Weekly Scheduled - Mon-Fri either... 8:30-5 - 9:30-6 & 10-6:30 * 1 Scheduled weekend every 4-5 Weeks This is an onsite role in Woods Cross, UT Pay: $19/hr 4-Month Contract to Hire Description: * Coordinates admissions for homecare, hospice, and skilled nursing facilities * Obtains pertinent pre-admission documentation from referral sources to determine if agency can care for the patient adequately * Review clinical documentation to determine level of care needed * Verifies physician orders * Develops and maintains relationships internally and with community partners * Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families Additional Skills & Qualifications: * High school graduate or equivalent * CNA/MA preferred, but not required * Demonstrate excellent customer service both in-person and over the phone. * Ability to work independently and as a part of a team. * Basic computer knowledge * Demonstrate excellent organizational and analytical skills. * Ability to multitask in a fast-paced environment * Good verbal and written communication skills * Minimum 1 year experience in healthcare * Knowledge of Skilled Nursing, Homecare, and Hospice criteria Experience Level: Entry Level - Intermediate About TEKsystems: We're partners in transformation.
