Temporary, Part-time Nurse Practitioner or Physician Assistant (Chula Vista) Contract completion bonus

Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls - enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

We offer an array of medical and chronic care services and we're backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

Homebase Medical is growing in the San Diego area - Chula Vista specifically. We are currently searching for 4 experienced Nurse Practitioners or Physician Assistants to engage in Medicare Wellness Visits for part-time hours for an 11-month contract at our contracted partner clinics and the occasional home visit.

**$1k contract completion bonus**

**Target start date is Dec 11th for training/onboarding and go live January 6**

**This is a part-time position of 3 days per week, 8-hour days (24 hours per week) **

Options:

1. 3 days a week Mon, Wed, Fri; 8-hour days: 24-29 hours
2. 3 days a week Tues, Thurs, Fri; 8-hour days: 24 hours
3. 3 days a week Mon-Fri dep on business need: 24-29 hours flexible with scheduling
4. 3 days a week Mon and Thurs with alternating Tues or Wed depending on company need; 8-hour days

Responsibilities include:

• Perform Wellness Visits in partner primary care clinics and facilities in the Chula Vista community.
• Drive to designated facility locations in your assigned geographic area.
• Review patient medical and family history.
• Review of current providers and prescriptions.
• Perform height, weight, blood pressure, and other routine measurements.
• Deliver personalized health advice.
• Review risk factors and potential treatment options.
• Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
• Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
• Provide referrals to appropriate medical resources.
• Utilize our EMR system to document patient visit/interaction accurately and timely.
• Knowledge of HCC coding.
• Other duties as assigned.
