Schedule Weekly Hours:

40 Gundersen Health System is looking for a full-time social worker for their adult Inpatient Rehab Unit. Opportunity to assist a variety of patient populations with discharge planning as they work towards returning to their home environment. What you will work:
  • 1.0 FTE = 80 Hours every 2 weeks
  • Monday - Friday, Day shifts
  • One holiday per year
What you will do:
  • Assessment and treatment planning
  • Discharge planning
  • Provide patient and family education and support

What you will need:
  • Bachelor's degree in social work or in a related field
  • WI Social Worker license
  • 1 year of previous experience in human services or health care
